How to refer
Complete our online referral form or call us directly on 1800 961 391. Our admissions team will contact your patient and keep you informed throughout.

Step 1 - Complete referral letter
Complete a referral letter with relevant clinical history and current medications.
Step 2 - Referral submitted to our Admissions Team
Submit our online referral form or send the referral via HealthLink (EDI: dphospit).
Step 3 - Admissions team reviews referral
Our Admissions Team will review the referral, contact the patient, and confirm the next steps.
Step 4 - Admission arranged
Once intake is complete, an admission date will be confirmed with the patient, between Mondays - Fridays.
